A numbered plastic label and padlock (if used) shall be attached to each isolation point.

When padlocks are used for isolation, the key to the padlock should be managed by the licensor.

Isolation should be secure to avoid accidental removal.

Even if the isolation is secure, if the "preparation" section of the permit requires "use of a personal padlock", the permit executor or specific operator is required to attach a personal padlock as required.

All personal hangings must be removed during shift or shift change.

Prior to the issue of a permit, inspections should be made to confirm that the required isolation has been implemented and is secure and effective.

If the valve is used as a means of isolation, only the following two safeguards are acceptable.

Lock valve in isolation position using steel chain or other locking device. The chain should be tightened to prevent the valve from loosening.

Use specially designed, movable valve interlock linkage. The opening of the interlock linkage shall be controlled by the licensor using a master key kept on the facility.
