Here's a scene illustrating the importance of LOTO: John is a maintenance worker assigned to a factory to repair hydraulic presses. The press is used to compress sheet metal, applying a force of up to 500 tons. The machine has multiple energy sources including hydraulic oil, electricity and compressed air. John follows the standard operating procedure and notifies the production manager that he intends to perform maintenance. He then followed the manufacturer's instructions to shut down the machine and isolate the energy source by cutting power, releasing compressed air, and draining hydraulic oil. He then applies lockouts to each energy source and tagouts to indicate that the machine is in service. John verified that the machine could not be powered back on by trying to turn on the power, press the operating button and activate the valve, all of which did not work due to the locking mechanism. John continued the maintenance work, deploying scaffolding to reach certain sections above the presses. After maintenance tasks are done, he carefully removes the equipment and performs a quick inspection to make sure everything is installed correctly. Production can resume after he and his partner clean up the workplace.  John's timely and accurate execution of the LOTO protocol ensured the safety of him and his colleagues during maintenance and prevented accidental release of energy from the machinery.
